Forbes Music Company LLC: Terms and Conditions

Billing & Payments

  1. All payments must be made by credit card on our website or by phone. We do NOT accept cash or checks. Payments given to instructor or made payable to instructor will NOT be accepted.
  2. All payments are required in advance of lessons. A credit card payment is required upon scheduling and booking your first lesson.
  3. Specific lesson dates, times, and instructor can ONLY be guaranteed for those clients who are enrolled in monthly or semester plans.
  4. For all new clients who do not elect to enroll in our semester plan, there is a one-time $25 registration fee.
  5. All necessary business, payment inquiries, and cancellation notices must officially be conducted through the office at Forbes Music Company LLC.

Monthly Billing

  1. Monthly Billing is calculated based on the number of lessons scheduled in the upcoming month. Clients will receive notification of pre-scheduled lesson dates no less than 5 days in advance of upcoming month. A minimum of two scheduled dates must fall within the month to qualify for the monthly billing rate.
  2. Clients can make modifications before the 1st of the month to the list of upcoming pre-scheduled dates. If no request for modifications is received by the 1st of the month, the client will be billed for all pre-scheduled lesson dates in the upcoming month.
  3. Federal Holidays are included in Monthly Billing. By request before the 1st of the upcoming month, Federal Holidays may be omitted.
  4. A student enrolling after the start of the month will be billed on a prorated basis. The total will be calculated based on the number of lessons remaining in the month.
  5. Payment will be automatically charged to the credit card on file on the 1st of the month for all scheduled lessons. Monthly Billing will reserve the day of the week, time, and instructor for each scheduled date..
  6. Any change in payment plan for the upcoming month must be made by the 28th of the current month. Notification must be made directly to the Forbes Music office by email or phone. Changes requested to or through an instructor will not be honored or recognized.
  7. If payment is declined, lessons may be canceled and your lesson reservations forfeited until payment is received and subject to availability.
  8. Only ONE make-up lesson is allowed per student cancellation per month, provided notice is given before 5:00pm the day preceding the canceled lesson date.
  9. Make-up lessons incurred while enrolled in Monthly Billing plan will expire after 60 days from the canceled lesson date.
  10. Summer scheduling: After June, all clients will automatically be switched to ad-hoc scheduling/pricing unless otherwise requested.
  11. If a client signs up for the Monthly Plan for lessons to take place on a recurring day of the week and time for the duration of the month, and at some point during the month chooses to change their availability, discontinue, postpone, or otherwise modify the arrangement of this prescribed schedule, the client is subject to forfeiture of those lessons whose date falls after the date of change, discontinuation, postponement, or modification.

Semester Billing

  1. Semester Billing is the prepayment, in total, of weekly lessons through the course of a 5 month semester.
  2. The fall semester is from September through January. The winter/spring semester is from February through June.
  3. All Federal Holidays will be omitted from Semester Billing. These holidays include Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, New Years Day, and Martin Luther King Day. Lessons may be scheduled with instructor on holidays on an ad-hoc basis and subject to availability.
  4. A 14-day winter break will be omitted from Semester Billing during the fall semester.
  5. A student enrolling after the start of the semester will be billed on a prorated basis. The total will be calculated based on the number of lessons remaining in the semester. The Semester Billing plan will not be available with fewer than 10 weeks remaining.
  6. Only TWO make-up lessons are allowed per student cancellation per semester, provided notice is given before 5:00pm the day preceding the canceled lesson date.
  7. Make-up lessons incurred while enrolled in Semester Billing plan will expire after 60 days from the end of the semester incurred.
  8. Summer scheduling: After June, all clients will automatically be switched to ad-hoc scheduling/pricing unless otherwise requested.
  9. If a client signs up for the Semester Plan for lessons to take place on a recurring day of the week and time for the duration of the semester, and at some point during the semester chooses to change their availability, discontinue, postpone, or otherwise modify the arrangement of this prescribed schedule, the client is subject to forfeiture of those lessons whose date falls after the date of change, discontinuation, postponement, or modification.

Individual Ad-hoc Billing

  1. For any lessons scheduled individually or on an Individual Ad-hoc Billing basis, credit card payment is required upon booking the lessons.
  2. Lessons will NOT be considered scheduled nor be reflected on our official calendar until payment is received in full.
  3. Days, times, and instructors are NOT reserved or guaranteed on a weekly basis for any clients who wish to schedule lessons on an Individual Ad-hoc Billing basis.
  4. A make-up lesson will expire after 30 days from the canceled lesson date.

Cancellations and Make-up Lessons

  1. All cancellation notices must be made directly through the office at Forbes Music Company LLC by phone or email. Cancellation notices made through the instructor will NOT be honored or recognized.
  2. Cancellation notice must be made by 5:00pm Eastern Time, the preceding day of scheduled lesson to be canceled in order to be considered sufficient notice. Any cancellation notice made after 5:00pm the preceding day of the scheduled lesson to be canceled will be forfeited by the client.
  3. In the event of inclement weather such as snow, hurricanes or other comparable conditions, the company reserves the right to cancel lessons without any financial obligation to the client. In this situation, a client can choose to make up the lesson or roll it over to a subsequent month.
  4. Teacher Absence or Cancellation: Any missed lesson due to the cancellation of the teacher will be made up or rolled over to a subsequent month without penalty to the student.
  5. Make-up lessons original canceled by client that are missed or canceled a second time due to a second student cancellation will be forfeited.
  6. No make-up classes or refunds will be given to an individual student absent for a group or multi-student lesson.

Sick Child Policy

The following illness policies will be strictly enforced, for the health, well-being, and safety of all concerned.

If the child shows signs of illness, the lesson will be canceled. Sick children will expose their teacher who can, in turn, expose other students. Since we are unaware of others’ heath situations or issues, this must be avoided at all costs. If other children or parents become ill due to such exposure, there is a possibility of causing irreparable harm. Because this is disruptive to other children and their families, your cooperation on this issue is extremely important.

Symptoms requiring lesson cancelation:

Please note: Calling either the office or teacher to inform of an illness but not canceling, will be considered a cancellation. Forcing a teacher or administrator to choose between health or loss of work is both irresponsible and disrespectful.

How would you feel if your lesson was immediately after another parent insisted a lesson take place when their child is sick?

Recitals

  1. Recitals are held twice per year, in December and May. Recital reservations are required in advance.
  2. Recital spots are available on a limited first come first serve basis and require a non-refundable $7 charge at the time of reservation.

Withdrawal

  1. For those clients on a semester plan: Payment is non-refundable after 10 weeks from the beginning of the semester. Should a student elect to discontinue semester plan within the first 10 weeks of the semester, a 50% refund of the remaining balance will be given.
  2. Clients taking monthly lessons that elect to discontinue with lessons remaining will NOT be refunded for uncompleted lessons.

Gift Certificates

  1. Gift certificates are available for purchase. Teacher availability may vary with respect to dates, times, and location. Please inquire about pricing and lesson availability.
  2. Lessons purchased with a gift certificate expire 180 days from date of purchase. No exceptions.

Hours and Holidays

Please contact us during our standard office hours, between 9:00am - 5:00pm, Monday through Friday. The office may be closed for business on all Federal Holidays and public school closings.

Client Responsibility

For a student 17 years of age or under, an adult or guardian 18 yrs old or older must be on the premises for the duration of the lesson. If an adult or guardian 18 years old or older is not present for the duration of the lesson, our instructor will not be able to teach that lesson, and that lesson will be forfeited.
